110-300-0145(1) Licensor observed broken items stacked against the fence in the outdoor play area, creating a hazard to children. 110-300-0145(12) Licensor observed the emergency exit pathway was being blocked in entirety by broken equipment being stored outside the licensed play area, in the exit pathway blocking gate. 110-300-0145(8)(9)Licensor observed a climbing structured placed against the gate to block off a missing slat of wood in the fence, creating a gap greater than 3.5 inches in diameter.
